Get Documents Ready
Please have the following documents ready to upload before you start registration:
Required Documents

Proof of Child's Name: This can be an official issued birth certificate, military ID, passport, state-issued ID or adoption decree.

Verification of Address: Provide a current home utility bill (gas, electric, water, garbage, land-line telephone, internet) that shows a recent date and service address, military base housing assignment documents, or rental agreement. Additional documentation may be required.

Immunization Record: Valid Washington State Certificate of Immunization Status (CIS) Form and/or Washington State Certification of Exemption (COE) Form.
For medically verified records from Washington state, there are a few ways you can obtain proof of vaccination:
- Option 1 - Sign up for MyIRmobile.com to view and print your family's immunization information.
- Option 2 - Request a complete immunization record from your healthcare provider, clinic, or local pharmacy. Most providers in Washington use the Washington State Immunization System. Ask your doctor or other primary care person if they can give you a complete immunization record for your child. They can print it from the Immunization Information System or provide records from their medical record system.
- Option 3 - Request a complete immunization record from your child's school. All public schools and some private schools in Washington have access to the Washington State Immunization Information System (WAIIS). Depending on staffing, schools may be able to print out immunization records for students if requested ahead of time.
- Option 4 - Request a complete immunization record from the Department of Health. Contact the Office of Immunization by phone at 360-236-3935 or 1-800-397-0337 or by email at [email protected].
For medically verified immunization records from another state, complete both of these steps:
- Obtain records from your out-of-state healthcare provider or your state's immunization database system.
- Submit the out-of-state records to your doctor or other primary care person. The provider can transfer them into the Washington State Immunization Information System or onto a Certificate of Immunization Status form.
